Instructions for Completing the FALL 2010 Textbook
Requisition Form:
1. Check to make sure the Department, Course
and Section number on the top of the form are correct. Please do not cross out
or make changes to this information. 2. In the column to the right of the book
information: Circle YES if you plan to use the exact textbook
listed on the form or NO if you choose to adopt a new book.
Circle REQ (required) or REC (recommended).
Recommended books include study guides, solutions manuals, dictionaries and
software reference manuals which may be helpful for students but are not
required for coursework. Please include all recommended titles listed on
your syllabus. 3. Enter all new adoption information in the
spaces provided. If a textbook package is adopted, please list all
components, noting which are required for coursework and which are not. Many
packages have been broken into components on the book order form to make this
easier. If a package is less expensive than the individual required
components, new packages will be ordered from the publisher. 4. Please note if the book has been used in
a prerequisite/co-requisite class so we can adjust order quantities. 5. Sign and date the form. 6. Please check with your Dean's
office for book order due dates. This is the book order that will be
placed for the FALL 2010 semester. Orders cannot be changed
after they are received by the bookstore. If you need additional time to
make a decision about your book adoption, please contact your Dean's office. If you have any questions about books or

SUPPLIES: If
there are any non-book materials that you require or recommend for your class,
you may include them on this form or email
Lyndsay Tagarel so we can make
them available in the bookstore and on our website. This includes
things like scantrons, blue books, flash drives, calculators, art supplies,
presentation board... SOFTWARE: The
Schoolcraft College Bookstore has many software titles available to students
and/or faculty at academic prices. Please contact
Brad Erisman for prices and
availability.
